Switch 2 Boost: Up to $500 Reward Card Switcher Offer Details

Reward Card: Get up to a $500 Mastercard reward card (the “Reward Card”) when you switch to Boost Mobile and port-in a new line with eligible postpaid service.

Requirements: 
(1) Previously had active wireless service from a competing wireless provider for a min. of 120 days and in good standing with a min. of 3 smartphone installment payments with an amount owed (payoff and/or early termination fee (ETF)) on a smartphone; 
(2) Purchase a new eligible smartphone from Boost Mobile on a qualifying 0% APR 36-mo. installment plan, purchase a new eligible smartphone from Boost Mobile bought at full price (“Upgrade & Switch”), or bring your own eligible, unlocked smartphone (“Keep & Switch”).
(3) Take and maintain the required plan for Upgrade & Switch (Plan: Unlimited Premium) or Keep & Switch (Plans: Unlimited+ or Unlimited Premium); and 
(4) Port-in new line from an eligible postpaid carrier (AT&T, Spectrum Mobile, T-Mobile, Xfinity Mobile, or Verizon) on a qualifying postpaid wireless line of svc (voice & data).

Reward Card Redemption:

• The Reward Card amount will be determined by the amount owed on the competing wireless provider smartphone (payoff or ETF) and will not exceed $500 per line; the new line must remain active for a minimum of 60 days from activation. Activation date starts the same day as Boost Mobile installment plan or start date of service for Keep & Switch and Upgrade & Switch. Keep & Switch device must be a smartphone; tablets, wearables, and hotspots are not included.
  
• The phone number used for card redemption must match the eligible ported number.
  
• Req’s most recent customer bill with current payoff balance (dated within one calendar month of activation with Boost Mobile) or final bill with early termination fees from an eligible postpaid carrier (AT&T, Spectrum Mobile, T-Mobile, Xfinity Mobile, or Verizon) must be uploaded via boostmobile.com/switchoffer within 60 days of activation of new line. Customers may submit one rebate form to cover all offers on one account. 
  
• The Reward Card(s) will be delivered to customers virtually within 1-4 weeks after upload of final bill and you maintain and pay for qualifying service on eligible line(s) for a minimum of 60 days from activation date. Customers will receive one Reward Card for all redeemed and approved offers. Card expires at month-end 6 months after issuance. For Cardholder Agreement, go to boostmobile.com/switchoffer. Card is issued by The Bancorp Bank, Member FDIC, pursuant to a license from Mastercard U.S.A. Inc. and can be used everywhere Mastercard debit cards are accepted in the United States, District of Columbia, US Virgin Islands, and Puerto Rico.  No cash access. The Bancorp Bank does not endorse or sponsor and is not affiliated in any way with any product or service offered by Boost Mobile.
  

Limits: One switcher Reward Card per eligible new line. 

Chargebacks: If you cancel your service or change to an ineligible plan prior to 12 months from the date of activation of your Boost Mobile service, you agree to pay Boost Mobile the full amount you received in the Reward Card (the “Chargeback”). For example, if you received $400, and you change your plan to an ineligible plan or leave Boost Mobile within 12 months, then you’ll be charged the full prepaid amount of $400.  If you cancel, stop paying your bill, or have your service terminated for violating Boost Mobile terms & conditions, Boost Mobile reserves the right to charge your payment method on file the full amount of your Reward Card.
